Liberty Veterinary Hospital is proud to provide you with a variety of forms to help simplify your visits.

Help us save time and enable us to serve you better by downloading these forms prior to your visit.  Please print and complete the following 4 forms and fax your completed forms to us at 513-755-9107 or bring with you at your first visit.  If you are a current client and have moved or have any information changes, please print and update the New Client Form.  If your pet has been microchipped, don't forget to update your contact info with the microchip company as well! (LVH offers HomeAgain)

We strive to make you appointment as timely and convenient as possible.  Here are some things that can help make your visit even easier:

  1. Please remember to bring copies of your pets previous records.  If you don't have them,  please call your previous veterinarian and have them faxed to us (513-755-9107) or let us know the name and number of your last veterinarian prior to your visit so that we may call to obtain them. 

  2. Please review our Payment Policy Form .  Payment is expected at the time of service.  We gladly accept any of the following forms of payment:  Cash, Visa, MasterCard, Discover, American Express and Care Credit ($200 min).  Checks are welcome when Social Security, Driver's license and current Credit Card numbers are all provided.

  3. Golden Buckeye Card holder?  Yes, we accept them!  Please show the receptionist your card prior to your visit to take advantage of this discount.  Select services receive a 7% discount.  Ask us for details!

  4. Please feel free to contact us (755-9700 x290) to verify your appointment, ask questions or get directions.  Directions are available here as well.

Rescheduling & Cancellation Policy:

To provide services as promptly as possible to all patients, we request sufficient notice for cancellations or rescheduling, so we may offer vacated slots to other clients.

Ø      Exams should be changed by 8am the day of your appointment.

Ø      Surgeries & dentals require 24 hours.

Ø      We do charge for appointments which are missed or canceled w/o such notice.

Ø      If you arrive more than 10 minutes late for an exam, you may need to reschedule

             your appointment in order to be courteous to other clients.

We appreciate your understanding & cooperation in order to effectively serve all clients & patients. 

Please refrain from using cell phones during your visit.

Prescription policy

If you need prescriptions for your pet(s), please request them during your visit, or within 1 week thereafter.  There may be a small chart review charge for prescriptions requested 1 week or more after your latest visit.   Please review our information on internet pharmacies here.  For your convenience, many of the medications we carry can be purchased from us through our website,  Liberty Vet Online Pharmacy

Duration of appointment

Appointments may vary in length due to the nature of the visit.  Initial visits may take 30 min to one hour.  Follow up appointments may require 20 min or less.  Technician appointments generally take 15 min or less.

Emergencies

Emergencies can sometimes be seen during normal business hours.  To help us make your visit more efficient, please call ahead to alert us you are on your way.  After hours, a doctor is on call to answer your questions and help give you some direction in possible treatment.  When warranted, the doctor or staff may recommend you go directly to the CARE Center, where 24/7 emergency care is available.  We recommend all new clients pre-register with them to help expedite any possible visits.  Healthy Pet offers some helpful information on Pet Emergencies to help you prepare.

Refer a Friend or Neighbor!

Our Referral Reward Program is set up so that the more clients you refer to our office within any 6 month period, the larger credit we will put on your account.  You can earn between $10-$20 PER CLIENT REFERRED!!! So, keep us in mind when someone you know is in need of veterinary services for their furry family members. We will do our utmost to provide them with the same caring, professional services that you’ve come to expect.
